SUMMARY Family Service League is seeking a full-time Social Worker to provide clinical services to youth and families through our Children and Family Treatment and Support Services (CFTSS) program. CFTSS is a team-based model which is intended to provide families individualized and flexible services by meeting with children in their homes and other community settings. CFTSS provides treatment to children ages 5-21 and enables a greater focus on prevention and early intervention by providing an array of services and the capacity to intervene earlier in children's lives. RESPONSIBILITIES The Social Worker will provide community based mental health treatment to a caseload of 20 children/youth and their families. Deliver services with the core values of CFTSS: "Family-Focused, Community- Based, Multi-Systemic, Culturally Competent, and Least- Restrictive." The Social Worker will provide evidence-based interventions that are tailored to the child's and family's needs. Complete treatment plans and implement services based on your assessment and the input of the family. The Social Worker will complete all documentation in accordance with CFTSS standards in an electronic medical record, including notes, assessments, and treatment plans. Collaborate with other CFTSS and Care Coordination providers by engaging in regular communication. Conduct crisis intervention as needed. Participate in team meetings. All other duties as assigned. QUALIFICATIONS LMSW, LMHC, LCSW, LMFT, or related licensed clinical degree required. Limited permit holders may apply. Minimum of 1 year of experience in a behavioral health setting and in a clinical capacity required.Prior experience diagnosing using the DSM V and experience with behavioral/therapeutic interventions for children, adolescents, and families required.Proficient computer skills, including Microsoft Office and Electronic Medical Records (EMR) required.Excellent verbal and written communication skills required.Valid and clean NYS Driver's License required."Family Service League is committed to the belief that all individuals deserve uncompromising respect.
