Job Description:We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support and guidance to individuals in need. This position requires strong interpersonal skills, the ability to work with diverse populations, and a commitment to making a positive impact on people's lives.Responsibilities:- Conduct comprehensive assessments of clients' needs and develop individualized care plans- Provide addiction counseling and crisis management services- Assist clients in accessing necessary resources such as housing, healthcare, and employment opportunities- Collaborate with other professionals, including social workers, behavioral health specialists, and workforce development agencies- Monitor clients' progress and make appropriate referrals for additional services as needed- Advocate for clients' rights and ensure they receive the support they require- Maintain accurate and up-to-date documentation of client interactions and progressSkills:- Strong knowledge of addiction counseling techniques and crisis management strategies- Familiarity with workforce development programs and resources- Understanding of behavioral health issues and the impact on individuals and families- Experience in social work or related field is preferred- Knowledge of Section 8 housing assistance programs is a plus- Excellent communication skills, both verbal and written- Ability to build rapport with clients and establish trust- Proficiency in motivational interviewing techniquesWe offer competitive compensation packages and opportunities for professional growth.
