They are looking for individuals who are experienced, dynamic, and motivated to contribute to their ongoing success. The Role: You will be an essential part of a collaborative team, utilising your expertise within the field to provide valuable contributions. Your role will involve managing chronic diseases, conducting clinical medication reviews, and working within professional boundaries. Duties: Enhance health outcomes by collaborating within the multidisciplinary primary care team. Offer specialised insights on medical matters, ensuring cost: effective prescribing and adherence to clinical guidelines. Conduct medication reviews and provide evidence: based education to colleagues. Assist with patients medication requests, discharge letters, and clinic communication. Provide support for self: limiting conditions and promote effective communication about medication: related concerns. Lead in implementing efficient repeat prescribing systems. Contribute to practice achievements in Impact and Investment Fund and QOF targets. Requirements: Previous experience in general practice and supporting acute and long: term conditions. Registered with the General medical Council. Undergraduate degree in shop. Proficient understanding of legal, ethical, and organisational policies. Enrolled or qualified through CPPE Clinical Pharmacist Training pathway. Strong clinical leadership and communication skills.
