Health and Wellbeing Practitioner (HMP Coldingley) : Full Time, Permanent : Bisley, Woking : 35 hours per week : A GBP 24,000.00 per annum. Benefits: Training opportunities and career development. The Role : Health and Wellbeing Practitioner (HMP Coldingley): This rewarding role involves managing a caseload and offering appropriate harm reduction advice and guidance, conducting backssments, and devising and implementing care plans in collaboration with the service user. You will conduct structured one: on: one sessions and lead group: work sessions, and ensure continuity of care for individuals transitioning from the prison back into the community. You will be based on: site within a team that delivers combined psychosocial and clinical interventions providing recovery: orientated Health and Wellbeing Services to service users aged 18 and over.
