Please Do Not Delete About the Service / Department Hestia's Mental Health Integrated VCS Service provides highly accessible and flexible non-urgent mental health and community integration support to clients within NHS Older Adults Community Mental Health Team (aged 65+) with moderate to severe mental illness. About the role Build a therapeutic relationship with clients (Older Adults) using a person-centred recovery approach Work collaboratively as part of the Older Adults Multi-Disciplinary Team with clients to develop their recovery plan and promote personal responsibility. Work with clients to help them identify their own meaningful recovery and personal goals drawing (using Dialog+ backssment) and utilising a range of recovery tools, techniques and experience To be the named key-worker for a number of clients Act a mentor and role model the ability to have a meaningful life for clients Connect clients to community groups and networks that enable them to participate in community activities and progress in their recovery journey Build relationships with the local community and statutory services and connect clients to those resources and support a ‘warm handover'Support the backssment and management of client care needs through completing, initial data collection, monitoring of clients progress, feedback, discussion, and reports and in care reviews with the clinical team Accompany service users, if appropriate, to appointments/meetings where they need additional support Record client clinical information on the BEH Trust's electronic patient records system (RIO) About you NVQ Level 4 in care or equivalent OR Minimum of two years working experience in a mental health setting Excellent written and verbal communication skills(Desirable) Experience of working with older adults, people with multimorbidity or/and dementia.
