The position requires a PGY1 residency and will actively participate in rounds for both ICU and other unit(s) as needed. In this position, you will serve as a clinical resource for providers and staff. You will also assist with antimicrobial stewardship program efforts, in addition to other listed duties and responsibilities that are central to our clinical pharmacy program. This position is full-time (40 hours weekly, 8-hour shifts) that primarily works day shift with an evening shift component and a weekend/holiday requirement; currently every 3rd weekend and 2-3 holiday shifts per year. D. degree preferred ASHP-Accredited PGY1 Pharmacy Practice Residency required ASHP-Accredited PGY2 Specialty Residency in Critical Care or equivalent experience (3 years critical care) preferred Critical Care Board Certification preferred Current licensure (or eligible to reciprocate) in the State of Maryland Principal Duties and Responsibilities: Round as a member of the interdisciplinary team to deliver comprehensive medication management for all assigned patients Assist in discussions with patients and/or family members to help make informed decisions regarding pharmacotherapy options Provide accurate, unbiased, timely, and cost-effective drug information to medical, pharmacy, nursing and other health providers Provide drug therapy related education to assigned patient populations Provide pharmacokinetic monitoring and therapeutic adjustments when a targeted medication is prescribed Review the patient medication history to determine which maintenance medications should be continued during the acute illness Prospectively evaluate all drug therapy for appropriate indication, dose, drug interactions, drug allergies, and monitor the patient’s pharmacotherapeutic regimen for effectiveness and adverse drug events (ADEs), and intervene as needed Educate patients and/or patients’ caregivers regarding medication therapies used to treat patients during and after acute illness Collaborate with the unit dietitian to recommend modifications as indicated to optimize the nutritional regimen Document clinical activities that include, but are not limited to, disease state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education and other patient care activities Act as a liaison between the pharmacy department and the interdisciplinary team to educate health professionals regarding current drug-related policies, procedures, guidelines, and pathways Evaluate specific medication therapies consistent with departmental policy (e.g. anticoagulation, aminoglycosides and vancomycin) Appropriately convert/manage medications per P& T approved pharmacist policy (e.g. IV to PO and renal dose adjustment) Verify medication orders in the Pharm Net computer database for assigned patient care areas Participate in the investigation and resolution of missing dose issues on the unit when needed. Provide routine stewardship activities targeted at anti-infectives and other medications, including those that may be high risk for adverse events, high cost concerns, and inappropriate utilization Participate in reporting ADEs to institutional committees and national programs (e.g., the FDA Med Watch program) Participate in identifying and submitting events via Report2 Learn (R2 L) system and providing follow up assessment. Collaborate with medical staff, nursing, other members of the healthcare team, and hospital administration to prepare for accreditation and to address any deficiencies identified About Christiana Care: Christiana Care is one of the country's most dynamic health systems, centered on improving health outcomes, making high-quality care more accessible and lowering health care costs.
