Adults Social worker: Hospital discharge : From GBP32 p/hr Nonstop is currently working with a well established local authority in the Slough area who are looking for an experienced social worker to join their Hospital Care Management team. You will be based out of the local hospital, allowing which will allow for a lot less travelling and a more structured day with less Out of Hours being completed. Responsibilities: The team is made up of experienced social workers and social care backssors who work alongside doctors, nurses, occupational therapists, physiotherapists, mental health services Work with a multidisciplinary team to facilitate appropriate, safe and timely hospital discharges for those referred to our service Benefits: Immediate start Condensed hours available Full time / contract : 6 month contract Competitive pay rate : From GBP32 p/hr No extra travel Supportive management Work within your contracted hours Requirements Please apply if you are a Qualified Social Worker with a minimum of 2 years experience in an adults disability/ mental health setting. You will be a Social Work degree and have a driving license.
