Registered Nurse (RN) Care Coordinator Case Manager Naval Hospital Twentynine Palms Manage care from diagnosis through all phases of inpatient and outpatient treatment until the patient's first normal follow-up appointment (usually one year post-treatment). Provide coordination of off-site radiation care, and medical oncology, plastic surgery, and breast surgery. Full-time, M-F 40hrs per week, 8hr shifts Possess a minimum of 2 years experience in the last 3 years as a case manager. DUTIES OF THE RN CARE COORDINATOR: Perform a full range of RN duties in accordance with the assignment under the task order, including: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians' orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support. Provide professional nursing care in a knowledgeable, skillful, consistent, and continuous manner. Work in a multidisciplinary setting to manage and coordinate the clinical and administrative activities for breast health over the continuum of care from wellness to illness; and to facilitate early detection, reduce the incidence, and reduce the mortality of, breast cancer. Utilize all available resources and means with an interdisciplinary collaboration to maximize the number of screening mammograms for the catchment area beneficiaries and target population. Some interventions will include the coordination and distribution of wellness letters and telephone or direct contact follow-up with patients who require mammograms. Use breast cancer risk assessment tools to assist the provider in identifying those patients at high risk for breast cancer and who require further and more frequent health care intervention. Collaborate with members of the health care team to assist patient who has breast-related health issues including but not limited to: making follow-up appointments and visits (in-house, other military treatment facilities, network); ordering and obtaining results of diagnostic tests; scheduling appointments; preparing packages (pathology reports, pathology slides, radiologic/imaging films); completing forms needed for care; ordering prosthetic services/bras/wigs; arranging resources and groups for emotional support; arranging transportation; arranging home health assessments and support for family members and significant others. Participate in performance improvement activities related to breast health such as on teams, committees, and in video/telephone conferences. Maintain and track measures of effectiveness for mammogram rate, provider education, and level of awareness in the target population. Provide and present data to appropriate individuals and/or groups such as the Tumor Registrar, Population Health Team, and the Breast Cancer Program Committee to facilitate improvement strategies and establish best business practices and standards of care. Coordinate publicity, employ marketing strategies, and collaborate with the command and air station Public Affairs Officers to facilitate advertisement and ensure maximum participation in educational and awareness activities for both the health care provider and beneficiaries/target population throughout the catchment area. Manage care from diagnosis through all phases of inpatient and outpatient treatment until the patient s first normal follow-up appointment (usually one year post treatment). Participate in professional organizations including participation in at least one annual national conference appropriate to job function to be funded by the Government within reasonable cost and availability. Plan, develop, and implement a comprehensive Breast Cancer Awareness Program. This program shall focus on breast care education and training for health care providers and breast cancer educational and awareness activities for the target population. This program can include using available resources from the command, community, and other military treatment facilities and should be based on learning needs. The program should include at least one class per year for the providers and one event for the target population/beneficiaries. Use a variety of computer software, including Microsoft Word, PowerPoint, Excel and Access to enter, retrieve, and query data, develop charts and graphs, interpret medical care criteria, and complete end-of-day administrative tasks. QUALIFICATIONS OF THE RN CARE COORDINATOR: Associates Degree of Nursing or higher Graduate from a college or university accredited by The Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing (ACEN). Possess current unrestricted license to practice as a registered nurse in one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands. Possess a minimum of 2 years experience in the last 3 years as a case manager. Possess computer skills sufficient to perform the duties of the position.
