CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farmworkers and their family members. Today, CHSI has expanded services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are under-insured or un-insured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota. WHO ARE WE LOOKING FOR? We are on the search for a full-time Medical Provider (MD/DO/NP/PA) to join our Grafton, ND team. DUTIES:Provides primary health care to patients in Community Health Service Inc. practice sitesPerforms outpatient procedures within the scope of primary care.May assist the Medical Director in the revision and implementation of practice principles, clinical protocols, and health care plans as needed.Consults with staff nurse practitioners and physician assistants as directed. Follows all applicable national patient safety goals in the provision of patient care.Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude. Leads the team in the team-based care model Provides care in collaboration with the center's team of physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants.Serves as a resource person for clinical staff regarding patient care issues.Establishes and maintains rapport with community agencies.Performs duties essential to the position and program. Demonstrates a sincere dedication and loyalty to the philosophy and mission of CHSI PHYSICAL REQUIREMENTS: (Initial by duties you are able to perform)In an eight hour workday, the employee must be able to: bend and crouch occasionally, sit continuously and stand or walk frequently. Employees must be able to lift up to 50 pounds occasionally. Employees must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements. REQUIRED QUALIFICATIONS:NP/PA with 2-3 years experience in an outpatient clinical setting. Valid state medical license/DEA in the jurisdiction of a clinic required.Basic Life Support (BLS) is required.Ability to diagnose and treat conditions within the scope of full-spectrum primary care.Knowledge and understanding of the principles and standards of clinical practice in outpatient medicine.Thorough knowledge of health promotion and disease prevention strategies.Ability to work as a team member, establishing and maintaining effective relationships with patients and other staff.Champions Health Equity for patients and demonstrates cultural humility in interactions with patients and staff. DESIRABLE QUALIFICATIONS:Experience in working in community health centers, public health, or in a similar type of program.Ability to communicate effectively in English. Bilingual provider preferred (Common languages served: Spanish, Somali, Arabic, Amharic)Knowledge and understanding of socio-economic and health problems of underserved populations are advantageous.
