Job Summary The Population Health Pharmacist will support Medicare Advantage quality performance for assigned physician practices by driving medication adherence performance. The Population Health Pharmacist will work closely with the care coordination team and with providers to ensure assigned patients are on intended prescribed therapies, understand their medication therapy, and receive appropriate adherence coaching. Core Functions The Population Health Pharmacist reports to the management staff. The pharmacy services team will also direct daily tasks. Working closely with other members of the Catalyst Care management team and assigned providers to ensure patients adhere to their prescribed medication therapies. Communicating effectively with patients and providers to overcome any barriers to adherence or concerns with their medications in a timely fashion to minimize medication use gaps. Providing physician practices with evidence-based suggestions for drug therapy alternatives when adverse effects or medication costs are identified as medication adherence barriers. Providing physician practices practical strategies to aid in medication adherence. Working with the clinical and dispensing pharmacy teams to keep them up to date on mutual patients' adherence levels and minimize gaps in medication therapies. Supervising technicians and pharmacy interns throughout the medication adherence monitoring process to ensure pharmacy standards and regulations are followed and patient safety is maintained. Supporting Care Managers in completing medication reconciliations for patients recently discharged from inpatient facilities (Transitions of Care). Ability to work within multiple platforms simultaneously to generate reports, verify claims data, attain and fill prescription refills, and ensure medication pick up. Serving as a drug information resource to patients, providers, and other interdisciplinary Catalyst care management team members. Providing analysis of drug adherence data to the network Performance Advocates, Catalyst Leadership, and partnering clinics/physicians, as requested. Provides telephonic follow up with members to provide medication reconciliation and medication monitoring to assigned patients Complete assigned tasks related to medication monitoring services. As directed and supervised by the Market Lead, assist in the attainment of member's goals. Provides medication monitoring services utilizing prescribed workflows Complete assigned appointments per the workflow Assess patient adherence behaviors Eliminate barriers to adherence Assess appropriateness of therapy provided by the pharmacy Document appropriately in Care Management and Pharmacy programs Work with a central or local pharmacy to obtain and process prescriptions. Communicate with providers regarding services provided Support the member education goals regarding their specific chronic condition based on the Plan of Care Support the evolution of pharmacy services delivery for key patient populations if applicable. Will professionally conduct all. Remains objective and delivers unbiased decisions in their medication monitoring review and recommendations Administers tasks as detailed in the member-specific Plan of Care. Completes clear and concise documentation in Care Management programs. Participates in weekly Complex Case Reviews. Complies with Customer Service expectations as applicable to the Department and Health Plan. Comply with all departmental policies and procedures. Participate in departmental and company in-services as appropriate. Recognizes and adheres to the scope/limitations of job requirements. Performs other related duties as assigned. Qualifications and Education Requirements PharmD TX state pharmacy license and/or assigned market license Strong patient engagement skills, including active listening, de-escalation intervention training, and ability to teach complex topics understandably. Strong organizational and time management skills, working with little supervision while handling numerous projects at once. Excellent written/verbal communication skills, especially the ability to communicate telephonically professionally and effectively. Computer literacy in Word, Excel.
