The Population Health Team at Granger Medical Clinic is currently seeking a full-time clinical pharmacist to assist our team with the healthcare transformation movement. The desired candidate will be an integral part of the interdisciplinary clinical services as part of a comprehensive management strategy for high-risk patients. Services will be provided to patients in the primary care setting focusing on complex medication management, management of chronic diseases through consults from physicians, Chronic Care Management (CCM), and prescription-related quality measures. The clinical pharmacist's time will be spent working in a clinic-based setting with some time spent doing telephonic outreach. This position will report directly to the Senior Manager of Population Health. This is an evolving position that allows the desired candidate to be a part of launching innovative clinical programs to enhance quality of care for patients across the healthcare spectrum. Knowledge and Skills: Extremely detail-oriented Able to work in a high stress environment and manage multiple complex responsibilities Exceptional written and verbal communication skills Superior analytical, organizational, and critical-thinking skills Consensus builder who strives to develop partnerships, teamwork, and good working relationships Essential Functions and Duties: Seek to optimize appropriate, effective, safe, and cost-effective medication regimens for the patients in their care Provide professional and compassionate care to patients Serve as a medication-related clinic resource for providers Assist with prescription-based quality metrics for Value Based Agreements such as: Medication adherence Statin use in patients with diabetics or cardiovascular disease Medication reconciliation post-discharge Participate in clinical programs such as: Chronic Care Management (CCM) Consult and provide patient education regarding medication management for multiple chronic diseases Diabetes Protocol Consulting on diabetes therapy for opportunities to maximize therapy, reduce patient cost, minimize side effects, etc. May also include recommendations for treatment of macrovascular or microvascular complications of diabetes that are considered standard of care Serve as a drug information expert to the integrated care management team and physicians who request it Participate in monthly quality team coordination meetings with medical director, clinical team, and program director Assist in the continued growth and development of the clinical pharmacist position Additional Functions and Duties May Include: Providing Medication Therapy Management (MTM) for patients Writing articles related to medication treatment updates or recommendations for provider newsletters Conduct Annual Wellness Visits (AWVs) Precept pharmacy students and residents Take active part in initiatives and/or organizations that further the practice of pharmacy in Utah Participate in quality improvement projects to improve clinical care or pharmacy operations Qualifications: Utah Pharmacist License PharmD from an accredited College of Pharmacy American Society of Health-Systems Pharmacists (ASHP) Accredited Pharmacy Residency or Board-Certified Pharmacotherapy Specialist (BCPS) Equivalents Preferred Granger Medical Clinic offers competitive wages and excellent benefits.
