Early Help Lead Practitioner required in Wakefield for full time, temporary position. Initial contract for 3 months with the possibility of extension. Apply NOW to be considered Early Help Lead Practitioner required in Wakefield to work as part of multi-disciplinary team supporting the delivery of the Early Help Offer for families. This position is: Full time - 37 hours per week Initial 3 month contract - with possibility of extension £16.12 ltd per hour Within this role you will: - The post holder will have core functions as a lead practitioner for Early Help, working with other agencies and directly with children and families to improve identified outcomes. - Case management of packages of support for families with multiple needs, undertaking backssments, developing plans of support, and taking a lead co-ordinating role, including co-ordination of intensive multi- agency support packages where needed and leading on multi-agency reviews of the support plans. - Rapid response to Early Help referrals received into the Integrated Front Door to determine the immediate needs of a child, young person or family and to support information gathering to enable appropriate allocation. - To report on outcomes as part of the Children and Young People’s Performance Framework. To ensure that services are delivered in compliance with relevant legislation, local and national policy and guidance.
