Position Summary Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within as existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals. Qualifications Education Valid Drivers' license and access to a reliable vehicle Service Delivery Skills, Expertise and Experience Minimum of 2 years' experience working in community, preferably with program target population Experience providing outreach with hard to reach populations Must have experience working with a vulnerable population Must have outreach and advocacy experience Must have the knowledge and understanding of the Housing First model, and Assertive Community Treatment (ACT) methodology.
